Admission Forms and Entrance Requirements for Undergraduate Programmes
MODE OF APPLICATION FOR UNDERGRADUATE PROGRAMMES
Applicants with the appropriate “A” Levels passes (Direct Entrants) seeking admission at CUHAS into the
Bachelor of Medical Lab Sciences (Code BU001), Bachelor of Science in Nursing Education (Code BU002),
the Bachelor of Pharmacy (Code BU003) and Doctor of Medicine (Code BU004) are required to apply through
the CENTRAL ADMISSION SYSTEM with the Tanzania Commission for Universities (TCU) using the guidelines
provided by TCU as detailed in pages 163-165.
Applicants with equivalent qualifications should visit CUHAS website, download and fill in the
application form and submit their applications directly to the College. Please attach copies of your
Form 4 (‘O’ Level), Form 6 (‘A’ Level) certificates, Birth Certificate and 2 passport size photographs
with a non-refundable application fee of Tshs. 30,000/- by postal order or paid through the University
Bank Account No. 01J1054045500 with CRDB.
Equivalent Qualifications (2 Year Conversion Programme)
Holder of Advanced Diploma in Nursing Education with at least THREE credits in the following subjects of “O” Level
secondary certificate: Biology, Chemistry, Physics, Mathematics and English.
Equivalent Qualifications (3 Year Programme)
Holder of Diploma in nursing with at least THREE credits in the following subjects: Biology, Chemistry,
Physics, Mathematics and English. They should have a working experience of at least 2 years. Preference
will be given for those who have attempted “A” Level.
UNDERGRADUATE COURSES:
- DOCTOR OF MEDICINE (MD) – 5 YEARS

Direct Entrants (Form Six)
Holders of Principal pass at “C” grade or higher in Chemistry, Biology or Physics, provided that the
other two passes are not below “D” at ADVANCED level. Preference will be given to applicants with
"C" grade or higher in Chemistry or Biology in that order.Consideration will also be given to
applicants with Principal passes at C grade on higher in Chemistry and a ‘D’ grade or higher
in Biology or Physics. [C, D (5 points from 2 subjects)]

Equivalent Qualifications
Holders of B. Sc. Degrees from recognized institutions, majoring in BIOLOGY or CHEMISTRY.
Holders of a Diploma in Clinical Medicine with principal passes at any level in Chemistry,
Biology or Physics, in the ADVANCED certificate examinations.
- BACHELOR OF MEDICAL LAB SCIENCE BMLS) – 3 YEARS

Direct Entrants (Form Six)
Holders of three Principal passes in Biology, Chemistry or Physics/Geography; provided 2 principal
passes are at D level or higher [D,D,E(minimum 5 points from three subjects)]
Consideration will also be given to applicants with two principal passes in Physics, Chemistry
or Biology provided that one of them is at C grade or higher [C,E (4 points from 2 subjects)]

Equivalent Qualifications
Holder of an appropriate Diploma/Advanced Diploma with three Principal passes at any grade in
Physics, Chemistry and Biology at “A” level.
Appropriate Diploma/Advanced Diploma with at least three credit passes at any grade in Physics,
Chemistry, Biology, Mathematics and English at “O” level and has passed Diploma/Advanced Diploma
with an average of B grade or above. Preference will be given to students who have attempted “A”
level.
- BACHELOR OF PHARMACY (B.PHARM) -

Direct Entrants (Form Six)
Principal pass at C grade or higher in Chemistry, Biology or Physics provided that the other two
subjects are NOT below D at (“A”) level. Preference will be given to applicants with “C” grade
or above in CHEMISTRY or BIOLOGY in that order.
Consideration will also be given to applicants with Principal passes at C grade on higher in
Chemistry and a ‘D’ grade higher in Biology or Physics [C, D (5 points from 2 subjects)].

Equivalent Qualifications
Holders of B.Sc. Degrees, from recognized institutions majoring in BIOLOGY or CHEMISTRY
OR
A Diploma in Pharmacy from a recognized and accredited Institution with passes at a minimum of “B”
grade in all Diploma subjects with three principal passes at E grade in Physics/Geography and
Chemistry and Biology at “A” Level provided a minimum pass of C in Biology and D in Chemistry
was obtained at “O” Level.
- BACHELOR OF SCIENCE IN NURSING EDUCATION

Direct Entrants (Form Six)
Holder of form-six certificate with THREE Advanced level passes in Physics, Chemistry and Biology;
at least TWO passes are at “D” or higher.

Equivalent Qualifications (3 Year Programme)
Holder of Diploma in nursing with at least THREE credits in the following subjects: Biology, Chemistry,
Physics, Mathematics and English. They should have a working experience of at least 2 years. Preference
will be given for those who have attempted “A” Level.

Equivalent Qualifications (2 Year Conversion Programme)
Holder of Advanced Diploma in Nursing Education with at least THREE credits in the following subjects of “O”
